Early Support Full Programme for Pre-School Children with Complex Needs from Birth to 5
This programme follows national Every Child Matters guidance and offers support to families of children birth to five who have complex health and learning needs.
All families of children in the Borough with additional needs or disabilities and aged 0 to rising five have access to the National Early Support materials, including a Family Pack, with extensive information booklets on a wide range of special needs and disabilities.
Eligibility
The programme is designed to support children aged 0 to rising five, living in Bromley, who require involvement from a large number of different agencies due to having complex health and learning needs. These needs are met by a combination of support from the Health, Education and Children's Social Care Services, e.g. Physiotherapy, Community Children's Nursing Team, Speech and Language Therapy, Pre-School Specialist Early Support Services, Portage, Sensory Support, etc.
A child's family or any professional working with the family can make a referral to the Early Support Pre-School Panel, which will decide if the child is eligible for support from this programme.
Details
The Early Support Full Programme provides:
- an initial home visit by a member of the Early Support Team;
- valuable information, including the Early Support Family Pack and information on local services and support;
- initial Joint Assessment, bringing together a team of professionals working with the family to support them and their child;
- a key worker who will, where possible, co-ordinate appointments, provide support and information and be a single point of contact for the family;
- a Family Service Plan to inform everyone of the agreed provision and pathways for each child;
- the family get ongoing support at regular meetings with the professionals involved in the child's care called Multi-Agency Meetings;
- a shared, confidential, Multi-Agency database to ensure that accurate and relevant information is available to all professionals involved;
- access to early advice, support and a key worker is also available for young children with significant social communication difficulties, through the SPEACS programme.
