Bromley Council provides or pays for a range of services for residents of the borough aged between 18 and 64 who have a physical disability and sensory impairment. Services are aimed to help them and their carers to receive the care they need to live as independently as possible. This includes people who are living with HIV, acquired brain injury, insulin dependent diabetes, uncontrolled epilepsy, neurological conditions e.g. (multiple sclerosis, motor neurone disease, huntingtons disease), hearing loss and other conditions that make it difficult for people to live independently without help.

Eligibility

We aim to support people to live as independently as possible in the community.

To ensure that services go to those in greatest need, who are at risk of losing their independence, we have an eligibility criteria for those who are seeking help from social services. Priority for services is given to those whose needs have been assessed as meeting the criteria.

Details

If the staff in Bromley social services direct (BSSD) think that  care services will be able to help you,  he/she will take  some details of the problems you are facing. A care manager will then contact you and your carer to arrange a time to visit you to assess your needs and agree with you the best way to help. In order to fully assess your needs the care manager may need, with your agreement, to work closely with agencies such as health service providers and voluntary organisations.

You may have to wait for an assessment. We will respond to requests for assistance according to priority of need. You will be told approximately how long you will have to wait.

The details of the assessment and what help will be offered will be written down in a ‘care plan’ and a copy given to you. The care manager will then arrange for any services that have been agreed to be provided. This may include occupational therapy, support in the home, leisure activities, education and training opportunities and accommodation.

If social services cannot help you the duty care manager will, wherever possible, advise you on where to go for the assistance you need. 

The  Bromley Association of People with Disabilities (BATH) website (www.bath-disability.org)  offer support including advice, information, the Holiday Respite Scheme, the Bromley Community Transport Scheme and the Independent Living Centre-Lewis House (demonstrating aids, adaptations and equipment designed to enable safe and independent living through Occupational Therapy and assessment).

Costs

There is no charge for an assessment of your needs to decide whether social services can help. However, depending on your financial circumstances you may be required to contribute to the cost of any services arranged for you by social services.

If you are in hospital and need help now or when you go home: Contact the Social Services care manager based at the hospital either direct or via the ward staff. Offices are open Monday – Friday from 8.45am to 5.00pm.
