Residential/Nursing home care charges 2021/2022 Re-assessment form

Please complete this annual re-assessment form providing evidence of the following showing your income and capital from 12 April 2021.  All of the information that you provide, must show revised income, capital and expenditure post April 2021 in order for an accurate assessment to be completed.

  • Pension breakdown letter from the Department for Works and Pensions
  • Bank, Building Society or post office statements for the last three months
  • Premium Bonds, Shares and Saving Certificates
  • Other income, which includes – Private Pensions and/or Premiums.

If any of your financial circumstances change, such as the amount of money you are receiving, you should inform us as soon as possible.
