Online forms: Lost or stolen Blue Badge declaration and payment
Please complete this form if you have lost or had your blue badge stolen.

Where a badge is declared to be lost, the badge holder or their representative must sign the declaration to this effect.
If the expired badge is found after issue of the new badge, it should be returned to the London Borough of Bromley immediately.
If the lost badge is discovered to be in use by the badge holder or a third party, the holder or their representative may be considered to be liable for any fraudulent use. Should this occur, a full investigation will result and a fine may be incurred.
Please note you will need to make a payment of £10 with this application for your replacement blue badge.
