This application is only for pupils with an Education Health and Care Plan (EHCP)
Before completing your application, please make sure you have read the Council’s SEN Transport Policy and the guidance notes for parents and carers.
Please ensure that you read and answer all of the questions to ensure that the SEN Transport team have the information to consider when processing the application. If there is missing information, this will delay the assessment of your child’s application.
Applications must be completed by the parent or carer that has parental responsibility for the child.
Completing this application does not guarantee that your child will be assessed as eligible to receive travel assistance.
Once your application has been submitted, the Council will assess your child against the SEN Transport Policy and provide you with a decision within 20 working days.
Please note that our busiest time for transport applications is between July and September. This means that if you submit your application after 1 July, we cannot guarantee that you will have your decision within 20 working days or that any awarded travel assistance will be in place for the start of the academic year in September
