Funded by the Innovation Fund, Bromley Council and NHS South East London ICS has commissioned Community Catalysts to carry out a consultation to capture the wishes, wants, and needs of children and young people with special educational needs and disabilities (SEND) to support their preparations for transitioning to adulthood.
What is the project about?
- We are having a Big Conversation in the borough about preparing for adulthood
- We want to talk to young people with SEND aged 16 to 25
- We also want to talk to parents and carers
- We want to understand what life is like when young people grow up and move into adult services
- This change is often called ‘Preparation for Adulthood’ or ‘transitions’
Why are we doing this?
We want to make support better for young people as they move towards adulthood. We want to know:
- What a good life looks like for you - now and in the future
- What is working well
- What is not working well
- What support is needed
Your views will help improve services in Bromley.
Who can take part?
We want to hear from young people aged 16 to 25 with SEND and their parents/carers.
How can you take part?
You can choose how you take part.
