This form is for help from the Crisis and Resilience Fund, funded by the UK Government.

The Crisis and Resilience Fund offers short‑term assistance to residents who need immediate support due to an unexpected event or urgent circumstance.

Any payment is discretionary, subject to eligibility checks and the information you provide, and may depend on available funding.

  • Crisis Payments offer short-term help for immediate essential needs following an emergency or unexpected change.  We may arrange a home visit to verify the crisis information provided so we can assess your needs and offer the most appropriate support. Any visit will be handled sensitively and respectfully.
  • Housing Payments offer extra help with certain rent and housing costs; for example rent shortfalls, arrears, rent in advance, deposits, or emergency housing costs; Please note you must be in receipt of Housing Benefit or the housing costs element of Universal Credit in order to receive a Housing Payment.

We use one form so you only have to tell your story once. Depending on your answers, you will only see the sections that apply to your situation.


Before you start, please have:

  • Your contact details
  • National Insurance number
  • A brief explanation of what has happened if you are applying for a crisis payment
  • Any evidence that helps explain your situation
  • Your bank details (sort code and account number) — if your application is successful, this is how we’ll pay any Crisis Payment and/or Housing Payment (if one is awarded)
  • Your most recent bank statements (covering the last 3 months) and any savings accounts for this period as well
  • Proof of income – for example, a benefits letter, screenshot of your benefits award, or recent payslips
  • Tenancy Agreement (if applying for a housing payment)
  • Rent Statement (if applying for a housing payment)
