How to complete and return the completed food business questionnaire
Please download and complete the food business questionnaire.
Do not forget to save the questionnaire on your computer when you download it.
Name the pdf file of the questionnaire using your business name. When you have completed the questionnaire, save it on your computer.
Then return the questionnaire to us by email, attaching the food business questionnaire to the email as a file attachment.
Please ensure that you use your business name in the subject field of the email when you return the questionnaire so that we know who the email is from.
Alternatively you can print the completed questionnaire and post it to the Food Team.
